Formal, national/regional decision-making bodies introduced to Canada, Australia and New Zealand in the early 1990s Much of Western Europe has followed (e.g. NICE, SMC, TLV, G-BA, HAS) United States was slower to adopt “4 th hurdle” mechanisms:
Due to absence of a single healthcare provider? Due to competitive nature of health care insurance?
Due to increased role played by consumers? Adoption has increased in recent years
