What is FMEA?

FMEA stands for « Failure Mode and Effects Analysis » and it is a step-by-step approach for identifying all possible failures in a design, a manufacturing or assembly process, or a product or service.

Scope

System analysis Functional analysis

Failure analysis Evaluate impact and likelihood of risks

Define and monitor risk controls

Evaluate measures to prevent and detect failure modes and/or failure causes. Monitor implementation and effectiveness and re-evaluate afterwards

Evaluate impact, likelihood of occurrence and detection of each failure mode

Analyse potential failure modes, failure effects, failure causes and correlation between them (failure web)

Analyse function of each system element and functional correlation (function web)

Define scope

Analyse system and break down into system elements
